The renewal form for Canadian passports can be printed from the Canadian Government website at http://www.cic.gc.ca/english/. Minors have separate forms.
Pictures for Canadian passports in Jerusalem should be taken at Strauss Photo on corner of Mordechai Ben Hillel and the Midrachov (Ben Yehuda). They are the only photo shop in the area that is certified by the Canadian Embassy.
